Description

Quality Improvement University ("QI University") is an educational venue established by the Atrium Health-Urgent Care Division to ensure that its physicians have access to standardized processes and medical evidenced based materials for its Advanced Clinical Practitioners ("ACP") to utilize for achievement of practice readiness and clinical competency for certain core practices and procedures required in the practice of urgent care medicine. QI University can be utilized for the fulfillment of the quality improvement process that is required by the North and South Carolina State Medical Boards, in the collaborative relationship between the primary supervising physician and his/her State Medical Board assigned ACP(s).

Objectives

  • Discuss 2 important questions to ask a pregnant patient who presents urgent care exposed to varicella
  • Discuss important background information when dealing with varicella exposure/infection
  • Discuss physical exam findings of varicella
  • Review complications of chickenpox with emphasis on the adult patient
  • Summarize Congenital Varicella Syndrome in regard to risks to unborn fetus
  • Discuss management of various scenarios for our pregnant UC patient who is exposed or has clinical chickenpox
  • Review large cohort study in JAMA supporting safety of Acyclovir
  • Review and concentrate on Rocky Mountain Spotted Fever and Lyme. Briefly discuss Ehrlichiosis and tick paralysis
  • Discuss what we do if patient presents with tick still attached and how to prevent ticks from attaching if going to be outside
  • Review when ticks are most infectous
  • Review RMSF and Lyme presentations. Discuss time course of illness and appearance of symptoms
  • Discuss how RMSF and Lyme are diagnosed
  • Review treatment of RMSF, Lyme, Ehrlichiosis and tick paralysis
  • Review when prophylaxis is indicated - review our May QI article supporting prophylatic treatment for Lyme if certain criteria are met
  • Clarify confusion supporting ondansetron use in pregnancy
  • Summarize stair step approach to treatment of vomiting pregnant patient
  • Apply latest study (JAMA 2018) regarding ondansetron use and risk of cardiovascular and cleft palate malformations in the developing fetus
