The Mecklenburg Breast Health Coalition was founded in 2013 and is hosting this forum to create community collaboration around breast health/breast cancer resources and important practices to improve time to care and ultimately patient outcomes.

The program will provide tools for providers to help guide patients to available breast health resources, such as the Breast and Cervical Cancer Control Program (BCCCP). The discussion will also highlight key barriers to care for community members, particularly individuals who are uninsured/underinsured.

Keynote Speaker

Donald K. Jonas, PhD

Don Jonas is Executive Director of Care Ring in Charlotte, who will be addressing Interprofessional Collaboration to Improve Health Care . Care Ring provides health and wellness services to vulnerable and at-risk populations through a low-cost clinic, Physicians Reach Out and Nurse-Family Partnership.

This program is offered FREE OF CHARGE to all participants who register through this Charlotte AHEC website with appreciations to the Grant provided by Wells Fargo.

Objectives

  • Recognize barriers to accessing care that negatively affect breast health/breast cancer patient outcomes.
  • Describe strategies to help vulnerable populations enter into and stay within the breast health/breast cancer continuum of care.
  • Create a network of connections among breast health clinicians to strengthen communication and create a community of practice.
  • Utilize resources presented at BC Connect to decrease time to care for the breast health/breast cancer patients and ultimately increase patient outcomes.
