Join us for a live Heart Failure Patient Education Webinar focusing on concerns IM/FM providers have in managing their heart failure population. The interactive setting will allow for real time responses as well as an opportunity for IM/FM providers to connect with a Heart Failure Specialist should they have additional questions after the presentation (a HF mentor). Participants will be provided resources, including provider tip sheets (HF patient care management, palliative care, HF documentation and coding, etc.) and patient education pamphlets.
