Description

Interprofessional collaborative practice has been shown to improve patient outcomes, decrease costs of healthcare to patients and improve population health. It is defined as multiple health care workers from different professional backgrounds working together to ensure the highest quality of care for patients, families, caregivers, and communities. In an effort to improve interprofessional collaboration, this case-based module seeks to educate healthcare professionals, educators and students on the variety of professions involved in an interprofessional team and identify the importance of interprofessional interactions between those professions.

Objectives

  • Describe, compare, and contrast the roles, responsibilities, preparation/education, and scope of practice for each member of the interprofessional team
  • Define and demonstrate the importance of interprofessional education and collaboration
  • Identify specific contributions of key members on the interdisciplinary team to support patient-centered care as part of an unfolding case study involving transitions of care across settings
