Application Information (3 STEP PROCESS):
Please make sure you have completed the below STEPS 1 and 2 in order to officially register through Charlotte AHEC to attend the Summit. Registration through Charlotte AHEC is STEP 3. Please use the information sheet provided to you by Teen Health Connection in STEP 2 to register through Charlotte AHEC.
STEP 1:
Complete the 2023 Empower: Teen Leadership Summit Application/Forms.
Visit the Teen Health Connection Webpage for links to the forms.
STEP 2:
Teen Health Connection will review your application within 3 business days and you, and your parent(s)/guardian(s) will be notified if you have been selected.
Applicants who are accepted will be provided an information sheet and link to officially register through Charlotte AHEC.
STEP 3:
Register through Charlotte AHEC using the information and link provided by Teen Health Connection. (When creating an account, for fields that do not apply to you please type NA.) Upon completion, you will receive a confirmation email.
