Course Description:
The First Steps ACP Facilitator Certification course is designed to help participants learn the skills needed to facilitate foundational ACP conversations with individuals who are healthy or have early chronic illness. This course is intended for a variety of professionals (social workers, nurses, health educators, physicians, clergy, and other care providers) and community volunteers. This course is appropriate for all levels of knowledge.
Course Requirements/Pre-Requisites
(Instructions for completing the following pre-course requirements will be emailed to you upon registration for this course):
• Completion of Respecting Choice ACP Core Curriculum Course (Registration Link is Below)
• Completion of the Respecting Choices First Steps® ACP Facilitator Online Modules (1-5);
• Complete “practice” Advance Directive document; submit to Instructor for review (this will be returned to participant);
• Review the Advance Directive document appropriate for your organization/community
