Target Audience

Respecting Choices Advanced Steps® ACP Facilitator Certification is intended for healthcare clinicians (nurses, social workers, chaplains, physicians) and others involved in assisting those with serious illness or frailty in making treatment decisions based on their goals of care.

This workshop is jointly provided by Charlotte AHEC Nursing Education and Your Care-Your Choice, a department of Atrium Health.

Course Description:

The Advanced Steps Advance Care Planning (ACP) Facilitator Certification course is designed to assist participants learn the skills needed to facilitate foundational ACP conversations with individuals who are in their last few years of life. This course is intended for healthcare professionals (nurses, social workers, chaplains, physicians, advanced practice providers) and other care providers. This course is appropriate for all levels of knowledge.

Course Requirements/Pre-Requisites

(Instructions for completing the following pre-course requirements will be emailed to you upon registration for this course):

• Complete Respecting Choice ACP Core Components Course
• Complete Respecting Choices Online Modules 1-5
• Read through the following:

1. Advanced Steps ACP Conversation Guide: For individuals with decisional capacity (RC 3115)
2. CPR Decision Aid (RC 0301)
3. POLST form (appropriate for your organization/community or distributed by ACP Instructor)

Learning Outcome:

Participants will be able to demonstrate effective communication skills and techniques for facilitating person-centered conversations for those living with advanced medical conditions and frailty for both in-person visits and telehealth.

Objectives

  • Define benefits of Advanced Steps conversation and identify appropriate target population.
  • Demonstrate the application of the person-centered decision-making framework to elicit preferences and decisions.
  • Create strategies to transfer individual goals, values, and preferences to a written plan.
  • Demonstrate beginning competency in facilitating Advanced Steps ACP conversations through role-play exercises.
