Nurses, Care Managers, Case Managers, Social Workers, Clergy, Nurse Practitioners, and other healthcare professionals who assist with Advanced Care Planning.

This workshop is jointly provided by Charlotte AHEC Nursing Education and Your Care-Your Choice, a department of Atrium Health.

Event Description:

The Advance Care Planning (ACP) Core Component Course is intended for professionals and healthcare providers (social workers, nurses, health educators, and physicians), clergy, volunteers, and others involved in providing ACP facilitation. This course is a prerequisite for attendance at First Steps, and Advanced Steps ACP Facilitator Certification Intensive courses.

The purpose of this course is to assist participants in learning effective communication and conversation skills and techniques for facilitating person-centered conversations. These conversations have application for in-person visits and telehealth.

Course Requirements/Pre-Requisites

(Instructions for completing the following pre-course requirements will be emailed to you upon registration for this course):

• Respecting Choices Online Modules 1-5
• Review Learning Tool: The Exploration Phase of ACP Conversations (RC 0125)
• View ACP Introduction video
• This course is a pre-requisite that must be completed prior to attending Respecting Choices First Steps®, or Advanced Steps® Advance Care Planning (ACP) Facilitator Certification Course.

How to Register for Other Classes:

  • Verbalize strategies to engage individuals and healthcare agents in advance care planning.
  • Demonstrate communication skills to promote person-centered conversations.
  • Apply critical thinking to discern what matters most to individuals.
  • Define benefits of First Steps conversation and identify appropriate target population.
  • Demonstrate the application of the person-centered decision-making framework to elicit preferences and decisions.
  • Create strategies to transfer individual goals, values, and preferences to a written plan.
  • Demonstrate beginning competency in facilitating First Steps ACP conversations through role-play exercises.
