The primary focus is to clarify that these improvements result from employing a multidisciplinary approach to treatment from primary care to access of specialty practitioners throughout process of evaluation, treatment and follow-up. The Committee’s expectation is improved patient outcomes through more efficient and effective coordination and communication of the plan of care.
