Description

This conference will explore a variety of topics relevant to improving patient transition of care. During the COVID 19 pandemic, virtual care plays an important role in ensuring continuity of patient care. Knowledge of available community programs and services is crucial in helping connect healthcare professionals and patients in need with available resources. Participants will be able to exchange ideas and recommendations through group discussion and use of case scenarios.

Objectives

  • Discuss the proactive management of COVID-19 patients across care continuum.
  • Explain the benefits of utilizing online community resources like Aunt Bertha and NCCARE360 to improve continuum of patient care.
  • Overview of the basics of Medicare for clinical case managers to help support patients as needed.
  • Review the current state of behavioral health.
  • Discuss how to implement a safety assessment and role of behavioral health screeners.
  • Discuss the palliative medicine and hospice care and how to identify benefits of each service to help avoid re-hospitalizations.
  • Integrate the concepts and knowledge learned from the class to generate group discussion through case scenarios.
