Social determinants of Health (SDOH) are the root causes of many health disparities. In this module, we'll take a look at SDOH, the negative health outcomes that are related to SDOH that impact U.S. communities, and potential projects, strategies and tools that these communities can implement to hopefully make a difference.

Target Audience
Community health workers, social workers, psychologists, licensed professional counselors, educators, school personnel, high school students, college students, health and human service practitioners, and other health care professionals.
