Social determinants of Health (SDOH) are the root causes of many health disparities. In this module, we'll take a look at SDOH, the negative health outcomes that are related to SDOH that impact U.S. communities, and potential projects, strategies and tools that these communities can implement to hopefully make a difference.

Objectives

  • Define social determinants of health (SDOH) and their importance
  • Explain how SDOH act as root causes for many health disparities
  • Identify a variety of different negative health outcomes related to SDOH that impact U.S. communities
  • Identify potential projects, strategies and tools that communities can use in efforts to reduce the negative health impacts of SDOH in their locale
