This event focuses specifically on provider education, engagement, and consultation with community partners to improve the access to quality care for patients living with sickle cell disease. The conference will also include quality improvement initiatives, opioid management, standardization of care across the state, and virtual educational activities. Also, nationally recognized speakers will share information on the new pathophysiology of sickle cell, disease-modifying therapies, and the cure with providers and community stakeholders.

Objectives

  • To identify and demonstrate evidence-based guidelines and quality comfort care for patients living with sickle cell disease
  • To assess complicated patient cases through virtual tele-medicine clinics with polling opportunities and case discussions to assess comprehension of complicated patient cases, evidence-based care alternatives, and disease modifying therapies
  • To employ peer-mentoring strategies to assist in cross-system teaching and consultation
  • To appraise current systems within their network based on EMBRACE Network support tools provided
  • To demonstrate decision support strategies that help guide pain management for individual, community resources for family engagement, and transition process measurement tools
