This event is jointly provided by Charlotte Area Health Education Center and Atrium Health Sanger Heart & Vascular Institute.

At this symposium, we will discuss strategies and opportunities for optimal transitions of care for the cardiovascular patient population across the care continuum. This event will include perspectives from different stages of care, pre hospital, during hospital, and post-acute care settings and how they apply innovation to patient care across the continuum.
