Target Audience

Respecting Choices® Last Steps® ACP Facilitator Certification is intended for healthcare clinicians (nurses, social workers, chaplains, physicians) and others involved in assisting those with serious illness or frailty in making treatment decisions based on their goals of care.

This workshop is jointly provided by Charlotte AHEC and Your Care, Your Choice.

The Last Steps® ACP Facilitator Certification Course is initiated as a component of quality end-of-life care for frail elders and those whose death in the next 12 months would not be unexpected. The Last Steps planning conversation is focused on goals of care in order to make timely, proactive, and specific end-of-life decisions.

This course will help professionals to develop the necessary communication skills to initiate end-of-life advance care planning discussions that help individuals establish medical and non-medical goals of care and convert them to medical orders that can be followed throughout the continuum of care utilizing the North Carolina MOST form.

Workshop Prerequisites
Completion of the Respecting Choices® Online ACP Facilitator Core Curriculum is required before attending the workshop. The online component includes interactive courses intended to help you learn basic ACP concepts and to come to the course prepared for a more robust classroom experience. Contact hours for the online prerequisite are available through Respecting Choices. Upon completion of the five individual modules, you will be able to download a contact hour certificate.

*Registration fees include the required online curriculum, as well as the on-site workshop.
Upon completion of registration, participants will receive a “key” to access the online curriculum.

Disclaimer
The term “certification,” as used in Respecting Choices® educational programs and products means that Gundersen Health System issues a certificate upon fulfillment of the following elements:

1) completion of a specified Respecting Choices educational program with standardized content and processes;
2) successful demonstration of skills associated with the specified program; and
3) if applicable, an 80 percent or higher score on a written examination associated with the specified program.
The Respecting Choices certification programs are not credentialed by a national accreditation body.

Objectives

  • Describe the goals of the Last Steps ACP program; a POLST Paradigm Program
  • Identify interview skills for facilitating Last Steps ACP conversations
  • Identify skills to assist individuals in making informed treatment decisions, to include CPR, limitations on treatment, and comfort care
  • Develop strategies to create a NC MOST form that accurately reflects an individual’s treatment preferences
  • Identify systems to honor the NC MOST form
  • Demonstrate beginning competency in facilitating Last Steps ACP conversations through role-play activities
