Description

This event is jointly provided by Charlotte Area Health Education Center and CHS Sanger Heart & Vascular Institute.

At this symposium, we will discuss strategies, tactics, and opportunities for optimal transitions of care for the cardiovascular patient population across the care continuum. This event will include perspectives from both acute care and post-acute care settings.

1) The nurses will learn best practices in specific areas that are new and there is an opportunity to improve knowledge for the cross continuum care of cardiovascular patients
2) Nurses will also learn how to use current data and strategies to identify areas where they can improve patient care and reduce readmissions.
