Description

Ambulatory Care Pharmacy Forum of NC Annual Meeting is a network for and led by ambulatory clinical pharmacists with an ongoing goal of providing networking, collaboration, and educational opportunities for likeminded pharmacists in North Carolina with an emphasis on practical ambulatory care topics and unmet public health needs.

In addition to providing networking opportunities for ambulatory care pharmacist, this knowledge-based CPE activity will provide the pharmacist with clinical practice updates on mental health, heart failure, pain management, and best practices for understanding and navigating recent Medicaid changes.

Program speakers from 1:00PM to 5:00 PM with networking reception to follow from 5:15 to 6:00PM.

The University Of North Carolina Eshelman School Of Pharmacy is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. The program ACPE #0046-9999-19-025-L04-P (Pharmacist) provides 4.0 contact hours of continuing pharmacy education credit. To receive CE credit, you must complete the CE attendance form and the evaluation of the program. Statement of credit can be viewed and printed in CPE Monitor. Statements of CE Credit will be processed in approximately 2 to 3 weeks.

Objectives

  • Discuss pragmatic suggestions for the appropriate outpatient management of chronic pain, including urine toxicological screening and pain contracts
  • List available resources to enhance a pharmacists’ role in working with patients with mental health disease(s)
  • Outline key changes from the 2017 ACC/AHA/HFSA Focused Update for the Management of Heart Failure guidelines
  • Discuss credentialing and privileging for pharmacists and how it contributes to the advancement of patient care delivery
  • Identify recent Medicaid changes regarding Clinical Pharmacist Practitioners and their impact on credentialing and billing
  • Identify the elements of an opioid medication stewardship program, and its application in chronic pain management
