A1R 1000
A1R/1000 was a Charlotte AHEC Asthma Initiative, funded by a grant through the Duke Endowment and in collaboration with CCHI. The mission of this 3-year program aimed to improve outcomes of care for all asthmatic children within the 8-county CAHEC region and to support the NC AHEC initiative of integrating practice-based, outcome-oriented approaches to CME. This collaborative program included interventions at the policy, health care system, and practice levels to implement a model of chronic illness care that involved all practices and communities in the CAHEC. We intended to spread this innovative approach throughout our region in order to play a key role in ongoing healthcare improvements in North Carolina.
This program was also part of a larger effort in which CCHI (formerly the UNC Children's Primary Care Research Group) worked in partnership with the NC Division of Medical Assistance and the NC Asthma Alliance to spread proven improvements in asthma care delivery to every North Carolina primary care practice caring for children.
