- Design Request and Billing Form - For posters, illustrations, and scan requests.
- For your convenience, AHEC Design utilizes a Drop-Off / Pick-Up bin located in the AHEC Library. The AHEC Library is located in the Medical Education Building (MEB) on the Carolinas Medical Center campus. Please call 704.512.6523 to inform us of a drop-off so that we may acquire your materials in a timely manner.
- CHS - Authorization for Release of Health Information Form (PDF, 125KB) - A general consent form to be used for image capture of Carolinas HealthCare System events and marketing materials (still photography and video).
