- Photography Request Form
- Request a portrait, event photography, etc.
- Medical Media Equipment Request
- For checking out audio visual equipment to be used on the CMC campus and equipment pick-up.
- Off-Campus Program Medical Media Equipment & Staff Request
- For programs not on the CMC campus that require audio visual equipment and staff support.

- The CMC Auditorium is owned and operated by Carolinas Medical Center. Any request for the usage of the Auditorium must come from CMC/CHS employees. Please call 704.355.5667 if you have questions about scheduling the CMC Auditorium for a program.
- CHS - Authorization for Release of Health Information Form (PDF, 125KB)
- A general consent form to be used for image capture of Carolinas HealthCare System events and marketing materials (still photography and video).
